Successful Treatment of Bulla with Endobronchial Valves
Emphysematous bullae are a complication of end-stage COPD. Patients with large bullae and poor respiratory function have limited treatment options. Surgical resection is a recognized treatment, but functional improvement after bullectomy is not satisfactory in patients with forced expiratory volume in 1 s (FEV1) < 35% predicted. When this 59-year-old male end-stage COPD patient was assessed, he...
متن کاملSuccessful treatment of a giant emphysematous bulla by bronchoscopic placement of endobronchial valves.
Surgical bullectomy is the treatment of choice for giant emphysematous bulla. We report a case of successful nonsurgical treatment with bronchoscopic placement of one-way endobronchial valves that are currently under investigation for the treatment of end-stage emphysema. In patients who are poor surgical candidates, this noninvasive bronchoscopic treatment may represent a valuable alternative.
متن کاملSuccessful Treatment of a Giant Emphysematous Bulla with an Endobronchial Valve in a Patient with Contralateral Lung Cancer
Patients with severe emphysema have a higher risk of developing lung cancer, and their surgical risk increases when emphysema is accompanied by a giant bulla. Here, we describe a patient who had an emphysematous giant bulla in the right upper lobe that was treated with an endobronchial valve placement. Subsequently, a cancerous lesion on the contralateral lung was successfully removed by lobect...
متن کاملCurrent status of bronchoscopic lung volume reduction with endobronchial valves.
INTRODUCTION Chronic obstructive pulmonary disease (COPD) is a major cause of morbidity and mortality worldwide. Emphysema is a component of COPD characterised by hyperinflation resulting in reduced gas exchange and interference with breathing mechanics. Endoscopic lung volume reduction using one-way valves to induce atelectasis of the hyperinflated lobe has been developed and studied in clinic...

142 was treated with bullectomy five years ago. He had been admitted to the hospital for three times with a similar presentation; a spontaneous, right basal pneumothorax and was treated with tube thoracostomy. He was conscious, with vital signs of arterial blood pressure 135/80 mmHg, the heart rate 92 beats per minute, respiratory rate 28 per minute and an oxygen saturation of 89% at room air. ...
